About Us

President and board member, Charles Perkins (Chuck), has an over 30 year background in nursing, with many years in home health care and hospice. During this time, Chuck realized a lack of financial ability for family members to travel to see their terminally ill family members before their passing. With the support of his peers, a team was created and the Final Journey Foundation was born! 

The Final Journey Foundation takes nominations from hospice and other end-of-life care facilities, and provides families with paid travel, transportation, and accommodations to these patients and families.  We are able to do this my receiving donations, raising funds, and by private and government grants. 

Final Journey Foundation strives to provide families with the opportunity to create lasting, final memories together with their loved ones.
